Campus Hazing Transparency and Reporting
In accordance with the Stop Campus Hazing Act (SCHA) Louisiana State University maintains a Campus Hazing Transparency Report (CHTR). This report provides information on confirmed hazing violations involving organizations formally recognized by the University and is updated biannually, as required by federal law.
LSU has a longstanding commitment to transparency related to organizational conduct. Since 2018, the University has maintained a publicly available Greek Scorecard for all fraternities and sororities, which includes disciplinary findings for organizational misconduct, including hazing. Prior to the implementation of the Greek Scorecard, LSU publicly shared chapter status information with the campus community during recruitment and intake periods. The Greek Scorecard builds upon these earlier efforts by providing a centralized and historical record of organizational accountability.
Each situation is unique, and all allegations are addressed on a case-by-case basis in accordance with University policy and applicable law. The University’s response to any specific situation does not create binding precedent but may inform the handling of similar matters in the future.

Currently, no organizations have been found responsible for hazing during the 2025 calendar year.

Organizational Learning Outcomes
- Education:
- SLG will update the advisor manual which includes proper communication protocol and reporting incidents, risk management and chapter operation resources.
- SLG will conduct live/recorded advisor training to inform them on sorority policies and procedures.
- The Membership Intake Process Guidelines: The Journey will be updated to include additional hazing training and provide clear guidelines on when and how new member practices should begin and how they should be conducted. Additionally, it will include the communication channels and how to report an incident if a member suspects hazing or unethical behavior.
- SLG will participate in national anti-hazing campaigns and initiatives to raise awareness and foster a broader cultural change.
- SLG will conduct regular assessments and surveys to measure the effectiveness of anti-hazing educational efforts and make necessary adjustments.
- SLG will continue to inform all members are aware of the sorority's strict no-hazing policy and the consequences of violating it through the following channels: SLG’s Annual Orientation, New Member Orientation, National Update Emails and Social Media.
- SLG will create a Standards and Ethics Committee training to assist members and advisors on navigating interpersonal conflict and improving sisterhood.

- SLG will share clear and specific expectations regarding intake paperwork submissions and changes to paperwork throughout the year through the following methods: SLG’s Annual Orientation, New Member Orientation, National Update Emails and Social Media.
- Regional Directors (RDs)
- RDs will host regional conferences and dedicate a workshop to anti-hazing education, ensuring participation from all chapters in their region.
- While conducting regular visits to each chapter, RDs will personally address the importance of a hazing-free environment.
- RDs will continue to establish clear and open communication channels between regional directors and chapter leaders to ensure ongoing dialogue about hazing prevention
- RDs will collect feedback from chapters regarding the effectiveness of anti-hazing programming and deliver this information to Headquarters for assessment.

- SLG will continue to require all alumni, advisory boards and active members to complete Journey 101 training which includes hazing prevention education to participate in journey education.
- SLG will create hazing awareness and prevention educational materials specifically for alumni.
- SLG will engage alumni in national anti-hazing campaigns and initiatives, fostering a sense of community and shared responsibility.
- SLG will educate Advisors regarding risk management procedures and require Advisors to sign code of conduct form prior to accepting advisory chapter positions.

Suspension/rescission of registration will be reflected on the Student Organization Scorecard. The Organization that has completed a period of suspension may be reinstated by complying with the Greek Life New & Returning Organization Policy and all outcomes designated as due prior to reorganization. The University will also ask that any national charter or affiliation that the organization has be revoked immediately. During this time, the organization will forfeit certain privileges, including:
The organization may not sponsor, cosponsor, or participate in any and all social, intramural, athletic, or other similar activities on or off campus; The organization may not solicit any new members or pledges; The organization may not initiate any new members; Organizations that maintain a residence or meeting facility on University property may not occupy or utilize that facility unless and until the organization returns as a registered studentorganization in good standing.
Additional stipulations of the suspension/rescission of registration or qualifications for registering in the future will be outlined below.
With respect to the information listed above, the following additional sanctions/stipulations apply:
- Prior to any request to return, Theta Chi Fraternity must be in compliance with the Greek Life New and Returning Organization policy.
- Prior to any request to return, the organization must receive approval by the Dean of Students or designee to develop and agree upon a two-year plan to ensure the chapter's successful reintegration into LSU's Greek community. Successful development of the plan is a pre-requisite for Theta Chi 's reinstatement as a registered student organization.
- Cease of all operation of Theta Chi Iota Pi chapter effective immediately until the approval for return via the Greek Life New and Returning Org Policy. This will include but is not limited to, the participation of any Theta Chi chapter members attempting to represent Theta Chi on the LSU campus or LSU-associated events.
- Prior to any request to return, Theta Chi must have a zero-balance owed to the University for any outstanding debt.
- The alumni chapter advisory team members must be selected and approved and must attend an educational session approved by Theta Chi Fraternity and LSU Greek Life on the topic of alumni advising on an annual basis for the four (4) academic years following re-organization. In advance of the educational session, Theta Chi Fraternity must provide agenda topics, content related to the expectations related to being an advisor of the chapter, and the presenter of the educational session to LSU Greek Life for approval.
- The current residents of the Theta Chi fraternity house must be vacated from the on-campus house by December 14, 2024. If any damages or violation of the cease of operations occur, they may be required to vacate sooner.
- Upon the reinstatement of Theta Chi as an RSO, the national fraternity shall not permit any member of Theta Chi Fraternity who was an active or new undergraduate member at the time of the chapter closure, to be involved with the new group of undergraduates.

In advance of the educational session, Theta Xi Fraternity must provide agenda topics, content related to the expectations related to being an advisor of the chapter, and the presenter of the educational session to LSU Greek Life for approval.
- Upon return to LSU, Theta Xi will provide a staff member, in addition to the House Director, 25 years of age or older, who will live in the facility and work with the fraternity on a full-time basis for not less than one academic year.
- The current residents of the Theta Xi fraternity house must be vacated from the on-campus house by January 31, 2024. If any damages or violation of the cease of operations occur, they may be required to vacate sooner.
- Upon the reinstatement of Theta Xi as an RSO, the national fraternity shall not permit any member of Theta Xi Fraternity who was an active or new undergraduate member at the time of the chapter closure, to be involved with the new group of undergraduates.
- The chapter will complete additional educational outcomes as identified in the Theta Xi Headquarters letter submitted to SAA on December 14, 2023. These outcomes should be completed within a year of returning as a registered student organization. The chapter will provide proof of completion of these outcomes.

Description of Violation: During Fall 2020, several SAE pledges and/or members "kidnapped" an active member, covered his head with a beer box, bound his hands and feet with tape, poured drinks and food items on him, and kicked the individual causing him to fall over. While these activities occurred, the member was asked organization specific questions such as "who are the founding fathers.

Organization Disciplinary Probation
This accountability status will be effective June 1, 2023 through May 31, 2025. During this time, the organization will be required to complete outcomes and may forfeit certain privileges which may include, but is not limited to, loss or limitation of social events or inability to participate in University events or activities. Disciplinary probation will be reflected on the Student Organization Scorecard.
With respect to the information listed above, the following additional sanctions/stipulations apply:
- Prior to any request to return, Sigma Alpha Epsilon must be in compliance with the
IFC Expansion Policy and Greek Life New/Returning Fraternity and Sorority Expansion/Re-Organization/Re-Chartering/Re-Activation
Policy.
- Sigma Alpha Epsilon must submit to SAA a copy of their completed Chapter Advisory Board Requirement Form showing that they have the required minimum of 3 advisors, most of whom live in the local area.
- Prior to any request to return, Sigma Alpha Epsilon must have a zero-balance owed to the University for any outstanding debt.
- Prior to any request to return, the organization must meet with the Office of the Dean of Students to develop and agree upon a two year plan to ensure the chapter's successful reintegration into LSU's Greek community. Successful development of the plan is a pre-requisite for Sigma Alpha Epsilon's reinstatement as a registered student organization.
- No socials, on or off campus, sponsored or co-sponsored by any perceived Sigma Alpha Epsilon third party that would represent the Sigma Alpha Epsilon undergraduate chapter may be held through May 31, 2023. This will include, but is not limited to, participation of any undergraduate Sigma Alpha Epsilon chapter attempting to represent Sigma Alpha Epsilon on the LSU campus or LSU associated events.
- The current residents of the Sigma Alpha Epsilon fraternity house must be vacated from the on-campus house by May 15, 2022.
- Upon reinstatement as a registered student organization, Sigma Alpha Epsilon will successfully complete all educational requirements of the SAE Fraternity Service Center during the probationary period.
- Sigma Alpha Epsilon Fraternity Service Center has agreed to conduct a membership review prior to the end of the suspension period, and as a condition of having the chapter suspension lifted. Requirements of this review will be conducted by the SAE Fraternity Service Center at the organization's expense. The membership review may not serve as an opportunity for the chapter to assemble in any official capacity.

- No request for reinstatement of registration will be considered prior to September 1st, 2023. No certified candidates or initiated members from the 2020-2021 roster can be affiliated with the undergraduate chapter or enrolled at LSU when the organization returns.
- Any evidence of an underground line or interest group in the name of Alpha Phi Alpha, or any affiliation with the fraternity and/or alias during the time of suspension, may result in the extension of the time of University Suspension/Rescission of Registration.
- Prior to the organization returning, the intake team must be approved by the Alpha Phi Alpha Fraternity, Inc. Louisiana District Director. The intake team must attend an educational session approved by LSU Greek Life, on the topic of advisory board training, on an annual basis for the four (4) academic years following re-organization. In advance of the educational session, Alpha Phi Alpha Fraternity, Inc. must provide agenda topics, content related to the expectations related to being an adviser of the chapter, and the presenter of the educational session to LSU Greek Life for approval.
- No socials, intramurals, athletic, philanthropic, service, fundraising or other similar activities on or off campus or sponsored, co-sponsored by or sponsored by any perceived Alpha Phi Alpha third party that would represent the Alpha Phi Alpha undergraduate chapter may be held through September 1st, 2023. This will include, but is not limited to, participation of any undergraduate chapter attempting to represent Alpha Phi Alpha Fraternity on the LSU campus or LSU associated events.
- The National Organization must be in compliance with the Greek Life New/Returning Fraternity and Sorority Expansion/Re-Organization/Re-Chartering/Re-Activation Policy.
- The current residents of the Alpha Phi Alpha fraternity house must be vacated from the on campus house by October 22nd, 2021. Current residents who may need assistance with housing can contact housing@lsu.edu for on campus living opportunities. The Alpha Phi Alpha Fraternity Inc. House Corporation will need to meet with the Executive Director of Residential Life to revisit the terms and conditions of the housing lease.
- Prior to any request to return, Alpha Phi Alpha must have a zero-balance owed to the University for any outstanding debt.
- In addition to this outcome, Alpha Phi Alpha will adhere to the plan set forth by Alpha Phi Alpha Fraternity Inc. National Organization as indicated below:
- The initiation process for the 10 certified candidates shall be completed by the District Director, Chief Dean, and appointed members from the local area Alumni chapters of Xi Nu Lambda and Beta Iota Lambda. The current members of the Nu Psi Chapter will not have any involvement in this process.
- The Nu Psi Chapter of Alpha Phi Alpha Fraternity, Inc., shall be suspended for a period of two years until August 2023. The chapter may return as early as September 2023 with under the condition that current chapter members and 2021 initiates are not enrolled at LSU. This suspension applies to ALL members, including the Spring 2021 initiates. This also means that the chapter and its members have forfeited all rights and privileges afforded to active members. This includes, but is not limited, to participation in the IMDP process, hosting of and participation in social, service, and educational events/activities (on and off campus), wearing of any fraternity paraphernalia or emblems/logos, and representing the fraternity on any level. This suspension also entails the immediate seizure of the chapter’s fiscal and fraternal assets by the Regional Vice President (or his designee).
- In addition to the suspension, the following sanctions are being imposed on the Nu Psi Chapter:
- The Chapter shall be placed on probation from September 2023 to August 2025. During
the probation period the chapter must meet the following conditions:
- All active members will be required to register and attend the District Conference and Regional Convention in each of the probationary years.
- Chapter delegate strength will be required to register and attend the General Convention in each of the probationary years when a convention is held.
- The Chapter will be allowed to only participate in service and academic related activities during the probationary years.
- All members will be required to attend the Fraternity’s Chapter Development Training
administered by the Area Director. The following modules must be completed during
the probationary period on an annual basis:
- Chapter Officer Training
- Review of Robert’s Rules of Order & Chapter Constitution & Bylaws
- Alpha Protocol & Etiquette Training
- Review of All National Programs
- Event Management & Social Activities
- Alcohol Awareness
- Risk Management & Initial Membership Development Process
- The chapter will be required to perform 50 hours of community service per semester.
- All members must participate and complete a comprehensive hazing training as approved by the University per academic year.
- All members will be required to attend any additional mandatory training as required by the Office of Greek Life at Louisiana State University.
- During the probationary period, an advisory team shall be appointed within the Alumni
chapters, Xi Nu Lambda and Beta Iota Lambda, which will be responsible for:
- Appointment of all chapter officers for the Nu Psi Chapter
- Oversight of all chapter operations
- Selection of new initiates
- The advisory team and area director will be responsible for administering IMDP.
- A member of the advisory team will be present during all chapter meetings and chapter administered events.

Description of Violation: Phi Kappa Psi was charged with and found responsible for coercive behavior, endangerment, failure to comply and hazing related to various activities in the Fall 2020 semester. Although not the only event at issue, the most significant of those events was held on or about October 18, 2020, was coordinated by the New Member Educator and resulted in the hospitalization of a new member with acute intoxication.

The Organization that has completed a period of suspension may seek reinstatement by complying with the registration requirements of the appropriate University department.
The University will also ask that any national charter or affiliation that the organization has be revoked immediately. During this time, the organization will forfeit certain privileges, including:
The organization may not sponsor, cosponsor, or participate in any and all social, intramural, athletic, or other similar activities on or off campus; The organization may not solicit any new members or pledges; The organization may not initiate any new members; During the period of suspension and until recognized as a registered student organization, the organization may not inhabit, occupy or utilize the house located at 3970 West Lakeshore Drive, or any other on-campus facility. Any current residents of the chapter house must vacate the premises by May 31, 2021.
Additional stipulations of the Suspension/Rescission of Registration or qualifications for registering in the future will be outlined below. With respect to the information listed above, the following additional sanctions/stipulations apply:
- Prior to any request to return, Phi Kappa Psi must follow the IFC Expansion Policy and Greek Life New/Returning Fraternity and Sorority Expansion/Re Organization/ReChartering/ReActivation Policy.
- Prior to any request to return, the organization must develop a strategic plan for how to address high- risk behaviors within the organization to ensure the organization's successful reintegration into LSU's Greek community. This plan must include specific objectives and key results. This plan must be discussed with and approved by the Dean of Students and representatives from Student Advocacy & Accountability and Greek Life.
- The organization must conduct a thorough membership review process by an individual outside of the organization for any current active members that plan to return to the organization upon its return to campus.
- No socials, on or off campus, sponsored or co-sponsored by any perceived Phi Kappa Psi third party that would represent the Phi Kappa Psi undergraduate chapter may be held through May 31, 2025. This will include, but is not limited to, participation of any undergraduate Phi Kappa Psi chapter attempting to represent Phi Kappa Psi on the LSU campus or LSU associated events.
- Prior to any request to return, Phi Kappa Psi must have a zero-balance owed to the University for any outstanding debt.
- Prior to any request to return, an alumni chapter advisory team must be selected and approved by Phi Kappa Psi Fraternity.
- Upon return to LSU, Phi Kappa Psi will provide a staff member, 25 years of age or older, who will live in the facility and work with the fraternity on a full-time basis for not less than one academic year
